Confidentiality and Privacy Policies

At CBHA, we are committed to protecting the privacy and confidentiality of your personal health information (PHI), including names and demographic information, clinical and/or financial information. CBHA complies with all federal and state privacy protection laws and regulations.  Your confidential information is only accessed by authorized personnel who are working within the scope of their job duties.  All information is kept in locked files or our secure computer system.

CBHA has established protocols to ensure that you are afforded your rights as defined under the HIPAA Privacy Rule with regard to your protected health information. The HIPAA Privacy Rule establishes a number of individual rights related to protected health information. CBHA makes every reasonable effort to honor those rights. CBHA gives serious consideration to all written requests by you and/or your personal representatives to exercise your rights concerning protected health information.

Notice of Information/Privacy Practices:

CBHA meets the definition of a business associate per the HIPAA Privacy Rules and, as such, agrees to uphold all provisions included in business associate agreements with covered entities. CBHA shall direct callers to contact their health plan to obtain a copy of the relevant Notice of Information/Privacy Practices.
