Disease Management Program Features
Referrals to the Program: Can be made by the primary care physician, specialist, EAP, or the patient themselves.
Initial Risk Assessment: The participant will be administered the PHQ-9 as well as a suicide assessment.
Mental Health Clinician: Each participant will be assigned to a mental health clinician who will follow the patient throughout the program and is available via of 800#.
Access: Participants can access a nurse 24/7 via of a toll free number.
Ongoing Assessments: Participants symptoms and medication adherence are reassessed with each telephone contact.
Education: Participants will be supplied education materials as well as community resources.
Translation Services: Services are available in English and Spanish
Physician Support: Prescribing Primary Care Physicians will be sent progress reports on symptom relief, medication adherence and patient progress. The CBHA mental health clinicians as well as the prescribing Primary Care Physicians have support of a psychiatrist through CBHA.
Closing Evaluation: Participants quality of life, symptoms of depression, medication adherence and satisfaction with the program will be assessed at the end of the 8 month program.
Reports: Monthly aggregate utilization reports and annual outcomes and satisfaction reports
