"Prescription For Recovery”… a disease management program offered by CBHA for depression and anxiety

The goal of CBHA’s “Prescription for Recovery” program is to offer a system of coordinated care with primary care physicians prescribing antidepressants and anxiolytics. A mental health clinician provides periodic symptom screens, interventions and education to patients enrolled in this program. During this eight-month program, patients are contacted at planned intervals to monitor symptoms, medication effects, treatment response, and work/life issues. After each contact the mental health clinician sends a progress report to the prescribing primary care physician to assess if the most effective outcomes are being achieved and if evidence based guidelines for depression/anxiety treatment are being followed. The Objectives of “Prescription for Recovery are….

  • To encourage screening, early identification, and early treatment of depression
    and anxiety
  • To empower members with depression/anxiety to manage their health and
    communicate effectively with their physician
  • To support the member and the physician with information, tools, and resources
    to help manage depression and anxiety
  • To monitor prescribed medications for effectiveness and adverse reactions
  • To monitor the severity of symptoms and quality of life issues
  • To improve outcomes and health for members with depression or anxiety
